5th, "Crystal Club VIP" Hologram, (This fifth issue reverse has "Flamingo Casino Kansas City" in black letters. The Crystal Club VIP Hologram card was only available for about 2 months before the casino was purchased by Isle of Capri. The coin-in requirements were supposedly $10,000,000! About 10-12 examples are presently known.)
