Error chip or "floor sweepings". Hot-stamp should have been applied to both sides, but one side is blank. By explanation, the hot-stamp operator may have been testing the temperature of the hot-stamp machine. A weak hot-stamp, like this, indicates the machine is not hot enough, but close. Accordingly, the chip is dropped to the floor and swept up at the end of the day as trash. Alternatively, the operator just neglected to run the other side of the chip through the hot-stamper.
